The 5th KH-8 flight, GAMBIT flight 41, was lost during launch from Vandenberg on 1967 Apr 26. It was followed by the final two KH-7 missions while the Titan 3B Agena D launch vehicle was requalified for flight. The Titan second stage malfunctioned and velocity was too low at Titan/Agena separation.
